Detailed Walkthrough
Talk to the Sick Hermit to learn that only a local witch can make the medicine to save him.
He gives you a key, so now you can go into the locked mine just to the South of him (you will have walked past it to reach him) and collect the herbs. There is a mixture of rats and bats in the cave, along with a couple of earth elementals, but nothing really dangerous. Work your way around the cave collecting the herbs until you have all 8.
Head back to Twainbrook - it's easiest to teleport with F12 if the Twainbrook Resurrection Monument is your currently active Monument. The Herb Woman that you need to talk to has her stalls just South of the fountain in the center of town. Head there and the quest will update. Now it looks as if the Herb Woman has a quest for you...
Talk to the Herb Woman to hear her offer - she is prepared to offer you a gold reward on the spot for the herbs, no more running required.
Accept
If you accept her offer then she'll give you the gold and a bit of exp, and that's the quest over.
Decline
If you decline her offer then you'll need to talk to her again. She's disappointed you didn't sell her the herbs, but she makes the medicine you need for the hermit.
Return to the hermit for your reward - no gold, but much more exp.
